Applications and Forms

These forms are in Adobe Acrobat format and provided for your convenience. To submit a form:

  1. Select a form from the list below.
  2. Forms must be PRINTED,FILLED OUT, and MAILED, EITHER to the ABCC, if it is an In-state License or Certificate of Compliance, OR to YOUR LOCAL BOARD, if it is a retail license.
  3. Please follow the instructions on the form carefully and fill it out completely. DO NOT LEAVE ANY BLANKS.

PLEASE TAKE NOTICE - THE ABCC HAS RAISED FEES, EFFECTIVE JULY 1, 2003 - PLEASE LOOK TO THE INFORMATION SECTION OF THIS SITE TO CHECK THE NEW SCHEDULE OF FEES.

  • License/Permit APPLICATIONS
  • NEW TO THE WEBSITE - THE FORMS LISTED BELOW CAN NOW BE FILLED IN AND PRINTED FROM YOUR COMPUTER TO SUBMIT TO THE ABCC
  • All Licenses and Permits are to be renewed ANNUALLY. Except for the "Agent, Broker, or Solicitor" application, new applications and renewals may use the same form. If you need to know the renewal date of your license, you may contact the Agency.
