Admissions Process for the Soldiers' Home in Chelsea
To be considered for admission to the Soldiers' Home in Chelsea, applicants must complete an application in its entirety and return it with all requested documentation. Please call the Admissions Office at 617-887-7146 to have an application mailed toyou or you may pick it up at the Admissions Office.
For Long Term Care (Nursing Home)
- Complete the application for the hospital in its entirety
- Provide a copy of the applicant’s discharge papers (also known as a DD-214).
- Provide copies of all current health insurance cards (front and back).
- If applicable, provide copy of any advance directives, such as Power of Attorney, living will, and guardianship order.
- Provide copy of proof of income (Social Security, Aid and Attendance, VA Retirement, etc)
- Provide proof of Massachusetts residency
- Once application is completed and all documentation attached, you may mail it or bring it to the Admissions Office at the Soldiers’ Home.
For Residential
- Complete the application for residential services in its entirety.
- Provide a copy of the applicant’s discharge papers (also known as a DD-214).
- Provide copies of all current health insurance cards (front and back).
- Provide copies of proof of income (Social Security, Aid and Attendance, VA Retirement, etc)
- Provide proof of Massachusetts residency
- Once application is completed and all documentation attached, you may mail it or bring it to the Admissions Office at the Soldiers’ Home.
Tour of Facility
If desired, you may request a tour of the facility. To schedule a tour of the hospital please call Mike Nappo at 617-887-7120 from 8:00 a.m. - 4:30 p.m. To schedule a tour of the residential section, please call John Steriti at 617-887-7150 from 7:00 a.m.- 3:00 p.m.
