Report H1N1 Vaccine Usage
Health care provider sites that have already enrolled in the H1N1 Vaccine Program are required to report on a weekly basis the total number of doses of H1N1 vaccine your site administered. Please use the following link to report your vaccine usage: Report H1N1 Vaccine Usage. As part of this on-line reporting system you will also be able to request additional doses of vaccine. Please note that requests are not a guarantee of additional doses. Vaccine is allocated based on the current vaccine priority groups, the number of doses available at the time, data reported in your initial registration, doses used to date, and your request for additional doses.
