Evidence-Based Programs


Many chronic conditions are preventable, treatable or manageable. Working with the Executive Office of Elder Affairs, the Massachusetts Councils on Aging and other community-based organizations, the Office of Healthy Aging brings workshops, training sessions and other health promotion activities to Massachusettsresidents. Activities empower participants to take charge of their health by focusing on life style changes that promote health and prevent injury.

My Life, My Health: Chronic Disease Self-management Program

My Life, My Health is a workshop that runs once a week, for six weeks, for two and a half hours each week. People with chronic health problems and those who care for people with chronic conditions attend six consecutive weekly sessions.

A Matter of Balance: Falls Prevention

Many older adults restrict their activities because they are concerned about falling. A Matter of Balance is designed to reduce the fear of falling and increase activity levels among older adults. The program includes eight two-hour classes.

Healthy Eating for Successful Living in Older Adults

Healthy Eating is for older adults who want to learn more about nutrition and physical activity for improved health. This six-week program focuses on heart and bone healthy nutrition strategies to help maintain or improve participants’ wellness and independence and prevent chronic disease development or progression.
