Applications for Massachusetts Controlled Substances Registration


In conjunction with the Secretary of Administration and Finance, and pursuant to M.G.L. c. 7, §3B, the Secretary has amended 801 CMR 4.02: Fees for Licenses, Permits and Services to be Charged by State Agencies.

The new fee for your Massachusetts Controlled Substances Registration, which became effective on July 1, 2003, is indicated on the application form.
