Provider information on reporting hepatitis C cases
On May 1, 2004, protocols for reporting hepatitis C infection changed. Boards of health will no longer investigate cases of non-acute hepatitis C infection. Instead, medical providers, their staff, or other providers (e.g. infection control practitioners) will be responsible for completing a case report form (CRF).
