CORI Forms and Applications
Effective Sept. 2, 2008 all applications for CORI certification must be submitted through the CHSB On-line Certification Application (OCA) program. The CHSB will no longer accept applications submitted by mail or fax. If your agency is NEW to the CORI certification process and has not previously been certified then you may submit your application by selecting the following link: http://coriweb.chs.state.ma.us/cass/terms.html
If your agency is applying to either RENEW or EXPAND your CORI certification and you have already been assigned an agency code, usercode and password then you may proceed with entering your RENEWAL or EXPANDED application. If you do not have a usercode and password please call the CHSB CORI Unit at 617-660-4640 prior to entering your RENEWAL or EXPANDED application. Please note, you will not need a usercode and password for the NEW application and do not need to call prior to submitting a NEW application.
Prior to entering a NEW, RENEWAL or EXPANDED application you will need the following information:
- Name and contact information for your company, including email address and phone number.
- License number (if applicable) for your agency.
- Name and date of birth for each individual that will request, review or receive CORI.
If your application is “Timed Out” please send an email to CORI.info@state.ma.us to notify staff of this message. Please note, if your application is “Timed Out” an application error has occurred and you will need to reenter your application. Please also note, you may not pause or save your application. Once you begin the entering the application you will need to enter all responses at one time.

Individual Agreement of Non-Disclosure and Statement of CORI Certification Compliance
Effective July 10, 2009 all Individual Agreements of Non-Disclosure (AOND) to be either added or removed from CORI certifications must be submitted through the CHSB On-line Certification Application (OCA) program. The Criminal History Systems Board (CHSB) will no longer accept paper AOND forms submitted by mail or fax. Below please find a detailed step-by-step guide on how to submit an AOND.
On-line Agreement of Non-Disclosure Submission Instructions:
- AOND forms are to be electronically submitted by the agency administrator. The system will not accept the usercode of an agency submitter.
- Prior to entering a AOND through the internet based OCA program you will need the following:
- Name, date of birth and position of individual to be added or remove.
- Social security number is optional you may instead submit any random nine digit number.(if applicable) for your agency.
- If your application is “timed out” please contact the CHSB legal department to notify staff as to the timed out application message.
- Please note, you may not pause or save. Once you begin entering AOND information you will need to enter all updates at the same time.
- Enter the following url into your internet browser: http://coriweb.chs.state.ma.us/cass/terms.html
- Review the Terms and Conditions and then select “I Agree”.
- Select “Access OCA.”
- Select “Update/Add Agreements of Non-Disclosure”
- Enter your agency code
- Enter your usercode
- Enter your password
- You will then view a listing for all AOND forms previously submitted for your agency. If you find that the individuals no longer are either employed by your agency or no longer have access to CORI please select the “remove” link next to their name.
- To add a new AOND, select “Add AOND”. You will then be prompted to enter the name, date of birth, social security number or other unique number and employee position. You will also check “submitter” and then select “I agree.” To submit the AOND select “Submit”
- The system will then take you back to the first screen. You may select “Add AOND” to add additional AOND forms. If you have submitted all your forms you may select “Continue.”
- Upon receipt of your AOND, the CHSB will review new AOND submissions and an email will be sent to the administrator to inform him whether the AOND has been approved or denied. The administrator may also log in at any time to check on the status of pending AOND form by accessing the same link listed for submission. Please allow between 2 to 4 weeks for processing.
CORI Certification Forms
This application should be used by those agencies or individuals not previously certified, whose need to access CORI is in the public interest (e.g. schools, camps, elder care), and whose purpose is not identified by the specialized applications listed.

- Executive Office of Health & Human Services (EOHHS) Providers Certification
The CHSB may authorize EOHHS agencies and vendor agencies to receive CORI regarding current or prospective candidates for employment or regular volunteer or training service positions in any EOHHS licensed, funded, or operated program where that candidate has the potential for unsupervised contact with persons receiving EOHHS services.

- Research Project Requests
The applicant for this type of access must demonstrate that the research being conducted is for a valid educational, scientific, or other public purpose. All applicants must provide a detailed description of the research project, specifying the type of CORI required and the reason that it is required. An approval for CORI certification for research is not an indication that the Board will supply the CORI data to be used in the project. The Board's approval or denial of the request will be based upon the purpose, design, and compliance with the requirements and guidelines contained in 803 CMR 8.00 Access to CORI for Research Purposes. The statutory basis upon which this access is approved is provided by M.G.L. c. 6, §173.

Any individual may request a copy of his/her own CORI. It is unlawful to request or require a person to provide a copy of his or her own CORI except as authorized by the CHSB, pursuant to M.G.L. c. 6, § 172.
This record requires a notary signature and must be mailed along with a business size, self-addressed stamped envelope to the CHSB. Walk-in or FAX service is not available.

- Advocates Request for CORI

An individual may need or want CORI information regarding a third party for various reasons. For example, an attorney may need to access their client's CORI, or a resource advocate may need CORI for community or employment placement purposes. It is unlawful to request or require a person to provide a copy of his criminal offender record information, except as authorized by the CHSB, pursuant to M.G.L. c. 6, § 172.

Parents may receive conviction and pending case data on a current or prospective child care provider who has the potential for direct and unmonitored contact with their children, and who are otherwise qualified for the position.

- Attorney’s Requests for CORI

Attorneys are permitted to access Massachusetts criminal offender record information (CORI) of both clients and non-clients with certain limitations and requirements.
CORI Relating to Client:
If you are seeking CORI relating to a client, the Advocate's Request for CORI form should be used.
CORI Relating to a Non-Client:
If you are seeking the criminal record of a non-client, the request will need to be made as either a Public Access Request, or for the purpose of witness impeachment or trial strategy.
Witness Impeachment or Trial Strategy:
The CHSB permits attorneys of record in civil, criminal and administrative proceedings to request criminal records for the purposes of witness impeachment or trial strategy. Access to the record is subject to the following conditions:
- Attorneys of record shall submit to the trial judge a motion requesting the approval of that judge to utilize the general grant of access to CORI, as provided by the CHSB, to Attorneys of Record. That motion shall identify by name and date of birth those individuals whose CORI is to be accessed as well as the type of CORI data for which access is sought (e.g. all criminal record information, conviction data only); and
- The attorney of record shall attach to the above-described motion an affidavit that the CORI sought is to be used exclusively for witness impeachment or trial strategy purposes related to that civil litigation, adminstrative hearing, or criminal case, and that such CORI as accessed shall not be disclosed to unauthorized persons in violation of M.G.L. c. 6, § 167 et seq, or 803 CMR 2.00 et seq.
Attorneys should send the approved motion, with a business size, self-addressed stamped envelope, and a check for $30.00 per requested name, made payable to the Commonwealth of Massachusetts. If the attorney is court appointed, include a copy of the Notice of Assignment of Counsel (NAC) form and the fee will be waived. Requests take approximately two weeks to process. All allowed motions for CORI data other than the Massachusetts court activity record (e.g. arrest reports) must be directed to the criminal justice agency having custody of the CORI data being sought.

- Publicly Accessible CORI

Any individual or organization may request that the CHSB provide a copy of another person's publicly accessible Massachusetts adult conviction record, pursuant to M.G.L. c. 6, § 172. For the adult conviction record to be "publicly accessible" the person whose record is requested must have been convicted of a crime punishable by a sentence of five years or more, or have been convicted of any crime and sentenced to any term of imprisonment, and at the time of the request:
- is serving a sentence of probation or incarceration, or is under the custody of the Parole Board; or
- having been convicted of a misdemeanor, has been released from all custody or supervision not more than one year; or
- having been convicted of a felony, has been released from all custody or supervision for not more than two years; or
- having been sentenced to the custody of the department of correction, has finally been discharged there from, either having been denied release on parole or having been returned to penal custody for violating parole for not more than three years.
A $30.00 fee per name is charged for all public access requests, regardless of whether or not a record is available and released.

- Model CORI Policy

This Model CORI Policy is provided to assist certified agencies with the compliance of new CORI Regulations (803 CMR 2.00-900 et seq) that are effective June 30, 2005. Certified agencies are required to maintain an agency CORI Policy and this Model Policy sets forth the minimum requirements of such policy.
