2009 License Examination Schedule
(All Exams begin promptly at 9:00 a.m.) You must be pre-registered for all license exams. Your completed application must be received by 5:00 p.m. on the deadline date listed below. If your application is received after the applicable deadline, you will not be allowed to sit for the exam. All license exams are offered at both Department of Fire Services locations: State Road in Stow MA and One Prince Street (Northampton State Hospital) in Northampton MA. Fax: (978) 567-3199 If you want to add an additional restriction to your MA Certificate of Competency please complete the add an additional restriction form and mail it to the Licensing Desk Address below. If you want to apply for an additional restriction to your company's MA Certificate of Registration, please complete the application to add a restriction to your company Certificate of Registration and mail it to the Licensing Desk address below.
To apply for a retest please complete the application for retest form and mail it to the Licensing Desk address below. If you hold a license from the Office of the State Fire Marshal and have recently changed your address, please complete the change of address form  and mail it to:Licensing Desk
