Optometry Renewal Instructions
The Massachusetts Optometry Regulations require every licensed optometrist to fill out a form every year listing all her/his regular practice locations. You must complete this requirement in order to renew your license.
To fill out the form online, use our Office Information Updates Required for Licensed Optometrists form, then provide the information requested on the security-protected pages that follow.
