By completing and submitting this Veteran of the Month Nomination Form, you agree that the information you have provided in this Form is true and accurate to the best of your knowledge. You acknowledge and understand that the information you have provided to the Office of the State Treasurer and Receiver-General (“Treasury”) and the Veteran’s Bonus Division on this Form, may be used by the Treasury and the Veteran’s Bonus Division in furtherance of the Veteran of the Month Program. You acknowledge that “in furtherance of the Veteran of the Month Program” may include, but shall not be limited to, use on the Treasury or Veteran’s Bonus Division Website, and in the local press.
