Pesticide Use Reporting
 Massachusetts state regulations mandate the completion of annual reports detailing the quantities of all pesticides used by licensed and certified pesticide applicators. Failure to submit a use report will result in the loss of eligibility for renewal of your license. Forms will be mailed to each pesticide applicator, or their company. Forms must be completed and returned to the Division of Crop Inspectinal Services, Pest Managment.
The deadline for submission of the 2011 Use Report is May 11, 2012.
