Notification Form: An electronic notification form

Cranberry Notification Form: A notification form for mailing to the Department of Agricultural Resources. Notifications must be made on a monthly basis to the Department. Notifications must be received by the Department within ten calender days of the end of the month in which applications are made. One form can be used for multiple applications in a single month.
