DBE once certified, and unless revoked will last for a period of 1 year and will automatically expire as of the date indicated in the letter, unless on or before that date, the certification is renewed.  Each company will receive a reminder letter 30 days prior to the expiration date.  The items that need to be submitted will be indicated with an X and year(s) indicated.


The Personal Net Worth of a company’s owner(s) will be evaluated every 3 years, and you will be notified and sent the forms in your reminder letter.

CHANGES IN CIRCUMSTANCE (structure, ownership, control)

You have a continuing responsibility to notify SDO of a change in any information that is relevant to the firm’s certification eligibility and to ensure that the information and documentation relied upon by SDO to certify or to maintain the certification of the business enterprise is accurate, complete and not misleading. You are required to notify SDO’s Director of Certification in writing of any change of such information or documentation within thirty calendar days. By way of example and not limitation, any change in ownership, control, ongoing or independence may be considered material. Failure to abide by the continuing duty requirements shall constitute grounds for the business entity’s decertification.


If you no longer wish to remain certified or the company is no longer in business, you must submit a written notification indicating that you would like the DBE certification to be withdrawn.

If the legal name or address of the company has changed, you must submit a written request on the company’s letterhead and signed by the owner(s) of the company along with a new W9 and Terms of Condition form   and submit with the request to:

To update your current certified description, or add additional naics codes you must request the change in writing. The request should include the current description and the new description you are requesting or the current naics and the new naics requested. Requests can be sent to Nedra White at Once the request is received, the file will be placed in queue for assignment to an investigator for further review.


If your company has both the State and Federal Certifications, when renewing each certification, you are required to submit information for both as each certification must have its own set of information although some of the information may be a duplicate.


All tax returns submitted either personal or business must be a complete signed copy including all schedules and attachments.  If you file a joint return with your spouse you may white out the social security number of the non-certified person.
