BI-ENNIAL RENEWAL – Every 2 years

M/NPO, M/W/NPO, W/NPO once certified, and unless revoked will last for a period of two years and will automatically expire as of the date indicated in the letter, unless on or before that date, the certification is renewed or recertified.  Each company will receive a reminder letter 30 days prior to the expiration date.  The items that need to be submitted will be indicated.

SUBSTANTIVE REVIEW

Certified companies that wish to remain certified must undergo a substantive review of their certification status with a SDO certification specialist who will re-evaluate the company to determine whether it continues to meet the applicable certification criteria. You will be notified when it becomes due for substantive review.  You will need to submit the applicable recertification application and all required information and documentation to SDO no later than forty-five (45) business days prior to the date of certification expiration (i.e., the recertification date).  At that time, a certification specialist will be assigned to evaluate your company and will make a report and recommendation to the Certification Committee (CC) on whether or not the company continues to meet the applicable certification criteria

CHANGES IN CIRCUMSTANCE (structure, ownership, control)

You have a continuing responsibility to notify SDO of a change in any information that is relevant to the firm’s certification eligibility and to ensure that the information and documentation relied upon by SDO to certify or to maintain the certification of the business enterprise is accurate, complete and not misleading.  You are required to notify SDO’s Director of Certification in writing of any change of such information or documentation within thirty calendar days.  By way of example and not limitation, any change in ownership, control, investment, ongoing or independence may be considered material. Failure to abide by the continuing duty requirements shall constitute grounds for the business entity’s decertification.

WITHDRAWING YOUR CERTIFICATION

If you no longer wish to remain certified as a M/NPO, M/W/NPO or W/NPO, you must submit a written notice indicating that you would like the certification to be withdrawn.  Send request to:

DECERTIFICATION

If your business has been decertified for more than 1 year, you must reapply for certification.  The first step in re-applying is to attend the pre-certification workshop (if business is in the State of MA).  If you are an out of State applicant you can take the webinar which is located on the main page of www.mass.gov/sdo.

If your company has been decertified for less than a year, send a written request to reopen the certification to

NAME CHANGES/ADDRESS CHANGES  (DO NOT EMAIL)

If the legal name or address of the company has changed, you must submit a written request on the company’s letterhead and signed by the owner(s) of the company along with a new W9 and Terms of Condition form   and submit with the request to:

UPDATE DESCRIPTION/CATEGORY EXPANSON

To update the current certified description, you must request the change in writing.  The request should include the current description and the new description you are requesting to have.   Requests can be sent to Nedra White at nedra.white@state.ma.us.  Once the request is received, the file will be placed in queue for assignment to an investigator.
