Enrollment and Change Form (Form 1-MUN) 
- For participating municipal employees and non-medicare Retirees and survivors changing coverage and/or canceling coverage. This form is also used for new hires enrolling in GIC Health Plan coverage.
Insurance Data Form (IDF)
- For participating municipal enrollees to add, change, and delete a spouse and/or dependent. Must provide corresponding documentation as outlined on the form.
