RMT Application
- to enroll in or change life and health coverage. NOTE: If your school district is not listed above, you will not be approved for GIC coverage.
GIC Insurance Data Form (IDF) 
- If you are already enrolled as a GIC RMT, and wish to add a spouse or dependent, send in this form along with a copy of the certified marriage certificate or birth certificate. NOTE: The following GIC plans are listed on the IDF form. However, RMTs are not eligible for these plans: Harvard Pilgrim Independence Plan, Harvard Pilgrim Primary Choice Plan, Tufts Health Plan Navigator, Tufts Health Plan Spirit, UniCare State Indemnity Plan/Community Choice and PLUS.
Dependent Ages 19 to 26 Enrollment Application
- Complete and return this form when you first enroll and want to add your dependent ages 19 to 26 to your coverage, during annual enrollment, or with proof of loss of other health insurance coverage.
