GIC Insurance Data Form (IDF) pdf format of    IDFform.pdf  - If you are already enrolled as a GIC RMT, and wish to add a spouse or dependent, send in this form along with a copy of the certified marriage certificate or birth certificate. NOTE: The following GIC plans are listed on the IDF form. However, RMTs are not eligible for these plans: Harvard Pilgrim Independence Plan, Harvard Pilgrim Primary Choice Plan, Tufts Health Plan Navigator, Tufts Health Plan Spirit, UniCare State Indemnity Plan/Community Choice and PLUS.

Dependent Ages 19 to 26 Enrollment Application doc format of    Dependent Ages 19 to 26 Application  - Complete and return this form when you first enroll and want to add your dependent ages 19 to 26 to your coverage, during annual enrollment, or with proof of loss of other health insurance coverage.
