Frequently Asked Questions
How do I file a Long Term Disability Claim?
As an employee of The Commonwealth of Massachusetts, you may contact Unum directly to initiate a Long Term Disability claim. You should contact Unum 30 days prior to the end of your elimination period (90 days from your date of disability).
To initiate a claim, dial toll free 1-877-226-8620 and select option 1.
Once you have initiated your claim by calling Unum, a Long Term Disability claim package will be mailed to your home address within one (1) business day. The claim package will include the following:
- Cover Letter with instructions;
- Claimant Statement;
- Attending Physician's Statement which must be completed by your doctor; and
- Employment Statement which must be completed by your Agency Coordinator
All paperwork should be submitted to the address or fax number indicated on the claim form.
When Unum receives all requested information, a claim number will be assigned and a Unum Disability Benefit Specialist will contact you directly within five (5) business days to establish next steps.
