Fill out the
Agency Funded Fax Registration Form

and fax the completed form with approval signatures to (617) 727-4331.
Reasonable Accommodation Requests
Definition: "Satisfaction of the unique, needs related to a person's medical, physical, mental, or developmental condition or disability."
PACE Registration
Students registering via PACE should list reasonable accommodations needed to participate in training events in his/her User Profile.
Agency Fax Form Registration
Students registering via the

should include any reasonable accommodations he/she on the registration form and inform your Department Training Liaison Officer (DTLO). DTLOs must notify HRD Training and Learning Development at 617-878-9827 at least two weeks in advance of a course start date of any reasonable accommodation needs to coordinate the request.
Cancellations
If a student needs to cancel his/her registration, notification of cancellation must occur prior to the published "Cancel Date." Students registering online may cancel their registration online. Students registered with agency funds may send a substitute, if necessary. The appropriate funding source will be charged for all confirmed course registrations if HRD Training and Learning Development has not been notified within the stated time.
DTLOs must notify TDG at least 72 hours in advance of the course start date if an employee needing reasonable accommodations will not be attending the course. The appropriate funding source will be charged for all confirmed course registrations if HRD Training and Learning Development has not been notified within the stated time.
HRD-Training and Learning Development reserves the right to cancel any course that does not meet the minimum attendance requirements. For additional information or help call 617-878-9827 or email HRDTraining@state.ma.us.
Substitutions
Agencies may substitute participants anytime before the start of the class. Please contact HRD-Training and Learning Development at 617-878-9827 when making substitutions.
Late Arrivals
The class instructor shall determine if a student who arrives late to a class will be able to complete the course.
Billing
Agencies will be billed after the completion of a course via the Intergovernmental IE/ITA process in MMARS. Agencies will be billed the full tuition rate for all students who fail to attend a confirmed training event, as well as, for training events students fail to complete.
