    Papers shall be filed according to the procedures set forth in 801 CMR 1.01(4)(a) through (e).

  • (5) Filing

    All papers filed with the Agency, its designee, or DALA should contain the name, address, telephonenumber and signature of the sender or Authorized Representative. Papers which do not contain all of thisinformation shall be accepted for filing if they contain sufficient identifying information so they can be placed in the appropriate file.
