Form
To request an addition, change or deletion of
Site and Building information, please click the
Site/Building
Information Form. You can print or save the
Form once it is displayed.
- User Information Form
To request an addition, removal or change of
privilege for a CAMIS User, please click the User
Information Form here. You can print or save
the Form once it is displayed.
- System Code Requisition Form
To request new CAMIS system codes, please obtain
the proper form from the System
Code Requisition Form. You can print or save
the Form after clicking on the item.
Please note that there are mandatory fields on the
forms. The completed form should be submitted to the
CAMIS Help Desk by e-mail CamisHelpdesk.DCAM@State.MA.US
or by fax to (617) 727-4043. You will receive notice
from the CAMIS Help Desk once the request has been
processed.
- Equipment Entry Form
The Equipment
Entry Form, this form is designed for CAMIS
Users to collect facility equipment data to be
entered into the system.
If no appropriate form is provided, you can still
make a request to the CAMIS Help Desk through fax
(617) 727-4043 or e-mail CamisHelpdesk.DCAM@State.MA.US.
Please follow the same format on the Request Form
to include Requester Information as well as requested
action.
