  • APPLICANTS DO NOT INCLUDE SOMWBA CERTIFICATION LETTERS

  • PROVIDING "NON-REGISTERED" REQUESTED CONSULTANTS

  • NOT SIGNING THE APPLICATION FORM

  • OWNERSHIP DOES NOT MEET LEGAL REQUIREMENTS FOR MA.

  • NOT SUPPLYING ALL REQUESTED CONSULTANTS

  • INSURANCE DATA NOT PROVIDED OR EXPIRED

  • PROJECT REFEREES NOT NOTIFIED OF APPLICATION
