Designer Selection Board Forms and
Instructions
Please be sure to use the lastest forms when applying
for State funded projects. It is the responsibility
of the Prime applicant to ensure all of their sub-consultants
also use the current application forms. Failure to do
so will be grounds for the application to be rejected.
At its meeting of May 11, 2005 the Designer Selection
Board of the Commonwealth of Massachusetts passed the
following motion:
ALL APPLICATIONS DUE ON OR AFTER JUNE 30, 2005,
WHERE THE DESIGNER IS SELECTED BY THE DESIGNER SELECTION
BOARD, SHALL BE SUBMITTED ON THE FORM ENTITLED COMMONWEALTH
OF MASSACHUSETTS DSB 2005 APPLICATION FORM AS
ISSUED BY THE DESIGNER SELECTION BOARD OR SAID APPLICATION
SHALL BE REJECTED AS BEING NON-RESPONSIVE.
PRIME APPLICANT IS RESPONSIBLE FOR ALL CONSULTANTS
USING LATEST FORMS.
The Form is available electronically at www.mass.gov/cam/DSB/DSB_App_2005.doc,
and in print form in the Central Register or by contacting
the Designer Selection Board.

to an advertisement for design services for a City or
Town and not as a result of DSB List click here.
Eligibility
Architects, engineers, planners, construction managers,
and related construction industry consultants who wish
to be considered for appointment as program consultants,
designers, or construction managers on state projects
under the jurisdiction of the Designer Selection Board
must submit current credentials in the form of the New
Master File Brochure 2005. A version of the Master
File Brochure 2005 in Adobe Acrobat File Format may
be downloaded here.

How to apply
Applicants should use the New DSB
2005 Application Form. Computer generated forms
are allowed provided they do not substantially deviate
from the DSB format.

The new DSB Sub-Consultant Acknowledgement (DSB
S-CA) is now the last page of the application.

Pursuant to Executive Order 390, the Commissioner of
the Division of Capital Asset Management, as designee
of the Secretary of Administration and Finance, has
developed goals and procedures
to encourage full participation by minority business
enterprises ("MBEs") and women businesses enterprises
("WBEs") (collectively "M/WBE¹s") on contracts for design
services entered into by Asset Management.
Design contractors need to file a SCHEDULE
FOR PARTICIPATION BY MINORITY BUSINESS ENTERPRISE -
PDF. (SCHEDULE FOR PARTICIPATION
BY MINORITY BUSINESS ENTERPRISE - DOC.) Design contractors
are also required to submit a LETTER
OF INTENT - PDF (LETTER OF
INTENT - DOC ) completed by MBE's who intend to
work on the project.
