In 2005, the Massachusetts Coalition for the Prevention of Medical Errors (the Coalition) was provided funding by the Betsy Lehman Center for Patient Safety and the Massachusetts Department of Public Health for a project aimed at reducing medication errors in long-term care facilities. The Coalition partnered with Masspro, the state's quality improvement organization and the Massachusetts Extended Care Federation, the state's largest long-term care provider organization, to collaborate on the most important outcome of the project: the development of a resource manual, the Safe Medication Practices Workbook.

The primary purpose of the workbook is to encourage long-term care facilities to use the tools and information provided to make the necessary assessments of their own medication management systems, in the interest of adopting best practice standards that will result in a decline in medication errors and foster a culture of patient safety.
