Dear Student Health Program Data Filer: The Division of Health Care Finance and Policy’s regulation 114.6 CMR 3.00 and M.G.L. c. 15A, §18 require Massachusetts institutions of higher education to annually submit information to the Division on their Student Health Programs (SHP) by November 1. Fall 2012 SHP filing applications are available via INET. For this filing, you are required to submit the following reports: - SHP Enrollment Survey for the 2012-2013 academic year
- SHP Benefit Survey for the 2012-2013 academic year
- SHP Performance Metrics Survey for the 2011-2012 academic year
This letter is a reminder that these reports are due to the Division by November 1, 2012. While many of the schools have submitted their data before the deadline, the Division will grant a two-week extension by November 15, 2012 for those who have not yet completed their filings. Instructions for filing each of these reports are available in the Division’s SHP web content at http://www.mass.gov/chia/consumer/basic-needs/student-health-insurance.html#Filing_Information. If your 2012-2013 academic year SHP benefits have not significantly changed from the 2011-2012 academic year, you may use the “Copy Survey” function to copy your answers from the 2011-2012 survey into the 2012-2013 survey and modify any information that has changed from the prior year. Please see page 24 of the Benefit Survey instructions for details. As a part of the SHP Benefit Survey data submission, you must also submit an electronic version of your SHP Evidence of Coverage, which can be e-mailed to the Division at HCF.Data@state.ma.us.
