The Center for Health Information and Analysis is collecting cost reports for the fiscal year ended in 2014 from all Adult Day Health (ADH) providers. Instructions to guide you in the completion of the report are available below. Regulations 101 CMR 310.04 and 957 CMR 6.00 require Adult Day Health providers to complete and file a cost report annually.

Please read all instructions carefully before completing the cost report.  To note, on Schedule E, Statement of Expenses, Column 4 Line 27 cannot be completed until you have first saved Schedule F Part 3.

If your agency does not participate in MassHealth (Medicaid) program, did not have a complete fiscal year or is a hospital-owned ADH, you will not be required to complete this report. An agency claiming an exemption on any of the above grounds must notify CHIA of the reason for its exemption by email to HCF.Data@state.ma.us.

If your agency has not registered with the CHIA’s INET service, you must do so to file the cost report.  See the link to INET Questions and Answers then complete the necessary forms to enroll as an INET filer.

All providers with fiscal year end dates between January 1 and June 30, 2014 will have a due date of November 3, 2014.  Those providers with fiscal years ending between July 1 and December 31, 2014 will have a due date of April 1, 2015. If you need an extension, please request one by following the process specified in the instructions.
