Adult Foster Care (AFC) Cost Report Information
Instructions
- We will only accept filings using Microsoft Excel 2003, Excel 2007 or Excel 2010. Do not use any version of Excel prior to 2003, as the data will not transfer correctly. Please save your cost report as a "Microsoft Excel Workbook," which is the default setting if you press "Save."
- All providers of adult foster care services that participate in MassHealth programs must file a cost report. Your agency MUST be registered with the Center for Health Information and Analysis to fulfill your reporting requirements. You are registered if your agency name appears under the "Agency Name" drop down list of the General Information section of the cost report. If your name does not appear in the "Agency Name" list, then you are not registered and you must follow the detailed instructions given in the AFC Cost Report Instructions. Completed cost reports should be emailed to hcf.data@state.ma.us.
- Financial statements or a tax return for the reporting period must be filed in conjunction with the cost report. These supplementary documents should be mailed to

Group Adult Foster Care (GAFC) Cost Report Information
Dual providers of both AFC and GAFC services must file separate cost reports.
