The Special Circumstances Application (SPCA) is used to submit applications for cases involving confidential individuals age 19 and under; abused and battered individuals; and medical hardship.

CHIA-INET User Agreement

Providers must identify the individuals who are authorized to complete the SPCA on the provider's behalf by filling out an INET User Agreement.

Individuals already registered for INET should fax the Center (Attention: Contact Management System Administrator) in order for the SPCA filing to be added to their INET account. Individuals not registered for INET should complete and submit an INET user agreement, relative to their provider site. See INET Questions and Answers for more information.
