APCD Data Release Regulations

Note:  New regulations replacing 114.5 CMR 22.00 will become effective Summer 2013.  The proposed regulations (957 CMR 5.00) are now available online at the following link. 
 

Application to Receive Data

CHIA will begin accepting APCD applications again in June 2013.  At that time, a new application form will be posted that conforms to CHIA’s new data release regulations.  Please register here to receive a notice when the new application form is posted.  In the meantime, here are the application forms that CHIA has previously used.  These are provided to give potential APCD applicants a sense of level of detail that will be required in their applications.
