File a Complaint


All complaints and written submissions should be addressed to the Executive Director.  It is not proper to address any submissions to the Chairman or to any other Member of the Commission.

You may use the below fillable form to file a complaint against a judge.  To fill in the below form, download it to your computer and then open it with the latest version of Adobe Reader.  A link to download Adobe Reader is provided at the bottom of the menu bar to the left.  Please use separate forms if you wish to file complaints against more than one judge.  

Please make sure that the complaint form is complete and accurate. You may use the statement of facts section to explain, in your own words, the circumstances that led to your complaint. Be as specific as possible in describing names, places, dates, docket numbers, and witnesses to the actions of the judge. List all the relevant events in chronological order. Use additional sheets, if necessary, and attach copies of any documents that support your statement. If you wish to include court documents, please do not send the originals.

Sign the complaint in the space provided. However, complaints may be anonymous. Make a copy of the complaint for your records.
