Petition for Guardianship of Minor

 

 

 

SEE G.L. c.201, Sections 1 and 5

 

The following must be filed with this petition:

 

Affidavit of Care and Custody 

(form available at the courthouse)

If petitioner is an agency then only one ward is

permitted on each petition.

 

In order to list more than one ward on a petition the

wards must have the same mother and father.

 

List the name(s) of the parent(s) and write "MINOR" if parent(s)

is/are a minor; if one parent is deceased list the survivor of them. 

If both parents are deceased then list the nearest

relatives of full age (over 18yrs. old).

 

The relationship of the petitioner to the ward must be stated

on the petition, i.e., aunt, grandfather, etc.

 

A bond must be furnished.
