Petition for Grandparent(s) Visitation

 

 

SEE G.L. c. 119, Section 39D

 

The following must be filed with this petition:

 

Affidavit of Care and Custody (form available at the courthouse)

If respondent father has been adjudicated in another court, a copy

of the judgment/order must be filed.

 

Affidavit describing nature of involvement and relationship

between grandparent(s)and grandchild(ren),

and description of harm child(ren) will suffer

if visitation is not ordered.

 

No filing fee is required.


Service of petition on respondents is to be made in accordance

with Massachusetts Rules of Domestic Relations Procedure Rule 4

 

Attorneys are required to file Uniform Counsel Certification
