THE CONFIDENTIAL JUROR QUESTIONNAIRE
HOW IS THIS FORM USED:
The Confidential Juror Questionnaire is a form that must be completed and brought with you when you appear for juror service. The information you supply on this form will be used by the judge and lawyers during the impanelment of a jury.
Except for verbal disclosures made during the impanelment of a jury, or unless a judge orders otherwise, the information you provide on the form will be kept confidential.
A blank copy of the Questionnaire will be mailed to you approximately ten days before your scheduled date of appearance. You will find it on the reverse side of the reminder notice that will be sent to remind you of you upcoming juror service. If you misplaced this notice, there is a version of the Questionnaire online that you may print and then complete.
