Annual Report for Hazardous Waste Recyclers
Annual reporting requirements apply to Class A recycling activities subject to permits, as well as Class B and Class C recycling activities. This report covers calendar year 2011 recycling activities and must be submitted to MassDEP no later than March 1, 2012.

Hazardous Waste Biennial Report
Large Quantity Generators (LQGs) as well as Treatment, Storage and Disposal Facilities (TSDFs) are required to complete this report. Forms must be completed using calendar year 2011 information and returned to the Massachusetts Department of Environmental Protection (MassDEP) by March 1, 2012.
A complete submittal consists of completed and signed paper copies of both the Massachusetts Notification of Hazardous Waste Activity Form and the RCRA Subtitle C Site Identification (SI) Form, as well as a complete set of federal Biennial Report Forms in either paper or electronic format as specified in the instructions below.

Complete this state version of federal form 8700-12 FIRST. You must submit a signed copy with your Biennial Report.

Federal Hazardous Waste Biennial Report Instructions & Form
Complete the applicable federal forms SECOND. You must submit the federal RCRA Subtitle C Site Identification (SI) Form along with the appropriate Generation & Management and Waste Received forms. The Offsite Identification Form is not required.
