Municipal Waste Combustors: Reporting Forms & Instructions

The table below summarizes the reports that facilities are required to file with MassDEP and provides links to the main pages from which to view them.

2	Annual Compliance Emissions Summary	Instructions MS Word Report MS Excel	Summarizes compliance stack testing results for all targeted pollutants, except mercury, during the reporting period. Submitted annually.
3	Data Capture for Continuous Monitoring Systems (CMS)	Instructions MS Word Report MS Excel	Demonstrates that a facility has collected sufficient data during the reporting period. Submitted annually.

5	Annual Compliance Emissions Summary for Mercury	Instructions MS Word Report MS Excel	Demonstrates compliance with quarterly and rolling annual average standards for mercury. Submitted annually.
6	Emissions During Startup, Shutdown or Malfunction	Instructions MS Word Report MS Excel	Summarizes specific operational events and corresponding emission concentrations/ parameter values during these events. Submitted annually.
7	Exclusion of Data	Instructions MS Word Report MS Excel	Lists instances in which data was excluded from reporting and explains why.
8	Corrective Action for Insufficient Data Capture	Instructions MS Word Report MS Excel	Lists any months and quarters during which the facility failed to collect sufficient data for any pollutant/parameter and explains why.
