Underground Injection Control (UIC) Fees and Forms

New Underground Injection Control (UIC) Registration Fees and Forms Effective October 08, 2004 - new UIC registration application fee schedule.

The new UIC Fee Schedule is:
WS-06a. - High - includes the following well types - 5A5, 5D4, 5W20 & 5X25
Registration Fee - $480.00
WS-06b. - Moderate - includes the following well types - 5A6, 5A19, 5F1, 5S23 & 5X27
Registration Fee - $240.00
WS-06c. Low - includes the following well types - 5A7, 5A8, 5B22, 5D2, 5G30, 5R21& 5X29
Registration Fee - $ 90.00
WS-06d. Pre-Closure form - for UIC wells not previously registered with the MassDEP
Registration Fee - $ 90.00

WS-06e Residential [for four units or less] the following well types (typical residential activities) are exempt from the UIC registration fee 5A7, 5D2, 5G30 & 5X18 but must still file the UIC Registration form with the UIC program.

Electronic Filing of Underground Injection Control (UIC) Registrations: The MassDEP online filing system recently added UIC registration filing forms to the eDEP menu. Although these forms were recently made live, they are still in beta testing. Please do not attempt to file through eDEP until this notice has been updated to indicate testing has been completed.

The UIC Program has revised UIC Registration Forms:

WS-06a, b & c registrations are all submitted on the same form. The applicant indicates which well type by checking on the top of the form a, b or c and submits the Transmittal Form with payment to the address on the Transmittal Form (a copy of the Transmittal Form should be sent with the UIC registration application to the UIC program/Boston).

Applicants submitting a UIC Pre-Closure form for UIC Class IV or V wells that have not previously registered with the UIC program must check both closure and registration on the top of the WS-06d-Pre-Closure Form. A Transmittal Form with payment is required for all Pre-Closure applications for Class IV or V wells that have not previously registered with the UIC program. The Transmittal Form with payment must be submitted to the address on the Transmittal Form (a copy of the Transmittal Form should be sent with the UIC Pre-Closure Form to the UIC Program/Boston).

Applicants submitting a UIC Pre-Closure form (WS-06d) for UIC Class IV or V wells that have previously registered with the UIC program must put the UIC registration number on the form (UIC registration number provided by MassDEP at the time of registering the well(s)). These applications (WS-06d) for closing registered UIC Class IV or V wells must be submitted directly to the UIC program in the appropriate region. There is no fee for WS-06d closure applications if the well(s) are currently registered with the UIC program and no Transmittal Form is required.

Residential properties of four units or fewer can submit their registrations on the NEW UIC registration form WS-06e. This application is used for residential activities only (stormwater, water purification system backwash, groundwater infiltration and geothermal heat pumps). This form does not cover activities associated with commercial or industrial activities at a residential unit. The UIC program has exempted the registration fee for these applications. No Transmittal form is required with this submittal.

• No UIC application requiring a Transmittal Form and Fee will be accepted for review until the Transmittal Form and Fee are received by MassDEP.
• Municipalities are exempt from the UIC registration fee.
• State Agencies are exempt from the UIC fees that are less than $100.00 (WS-0S c & d).

We will no longer accept old UIC Registration forms or UIC Pre-Closure form.
