Duplicate Certificates

If you have lost your Massachusetts Hunter Education certificate card, you can request a duplicate via our online Duplicate Certificate Request Form. You can also contact the Program at (978) 632-7648.

Please include the following information when you make your request:

  • Hunter Education CertificateFull Name
  • Date of Birth
  • Year You Took the Course
  • Location Where You Took the Course
  • Your Address When You Took the Course
  • Your Current Mailing Address
  • Your Daytime Telephone Number
  • An E-Mail Address (if you have one)
