The taxpayer can request a copy of any tax return, schedule or other supporting document that has previously been filed with DOR with Form M-4506. Generally, DOR retains copies of returns for six years.

A valid Form M-2848 - Power of Attorney must accompany any form request not signed by the taxpayer who has signed the original tax return. There is generally no fee for copies of most tax returns. However, a photocopying fee of 20 cents per page may be imposed for large returns such as estate tax returns.

To fill the request will take approximately four to six weeks.

Requests should be sent to:

Massachusetts Department of Revenue
Taxpayer Service Division
PO Box 7010
