Numeric Values for Scope and Severity Ratings


The alphabetic scope and severity (S/S) ratings and their corresponding numeric values are presented in the Scope and Severity Tables below. 

The first matrix shows the numeric values assigned to deficiencies from surveys completed after January 1, 2007 which are among the items used to determine Substandard Quality of Care.  For example, the Scope and Severity rating of "D" is assigned a numeric value of 1, and the rating of "G" is assigned a value of 3.

This matrix shows the numeric values assigned to deficiencies from surveys completed after January 1, 2007 but are not among the items used to determine Substandard Quality of Care.  In this instance, the Scope and Severity rating of "D" is assigned a numeric value of 1, and the rating of "G" is assigned a value of 2.

This matrix shows the numeric values assigned to all deficiencies from surveys completed before January 1, 2007.  In this instance, the Scope and Severity rating of "D" is assigned a numeric value of 4, and the rating of "G" is assigned a value of 7.  Also, note that a weighting factor has been assigned to each rating contained in the table.
