Hazardous Waste Transporters
- BWP HW 05 - License to Transport Hazardous Waste, Including Mixed Waste
This license is required to transport hazardous wastes from generators to authorized Treatment Storage Disposal Facilities.
- BWP HW 06 - License to Transport Class B (4) Regulated Recycled Materials (RRM)
Companies wishing to transport precious metals that are Class B(4) regulated recyclable materials are required to apply for a license.
- BWP HW 14 - Hazardous Waste Transporters License Renewal
Any party that wishes to continue transporting hazardous wastes beyond the expiration date of its current license is required to apply for renewal.
- BWP HW 15 - Renewal of Class B (4) Regulated Materials Transporter License
Any party wishing to continue transporting precious metals that are Class B(4) regulated recyclable materials is required to renew its license.
- BWP HW 27 - Permit Modification for Transporter License
Hazardous waste and precious metals transporters can use this form to modify their licenses without needing to apply for new ones.
- BWP HW 28 - VID Fee for Licensed Hazardous Waste Transporters
Hazardous waste transporters are required to apply annually for Vehicle Identification Devices (VIDs) and to pay to the MassDEP VID Fees. These fees are assessed according to the weight of manifested hazardous waste picked up and/or dropped off in Massachusetts during a 12-month accounting period ending the previous March 31.
- National Hazardous Waste Manifest Form
As of September 5, 2006, all Massachusetts generators and Treatment, Storage and Disposal Facilities initiating shipments of hazardous waste and waste oil must use the new national six-part manifest form required by the U.S. Environmental Protection Agency (EPA).
Hazardous Waste Treatability Studies
- BWP HW 07 - Approval of Hazardous Waste Treatablity Studies
This approval authorizes and sets conditions for treatability studies using samples of hazardous waste generated on-site or collected from off-site.
Hazardous Waste Treatment, Storage or Disposal Facilities
- BWP HW 09, 10, 11 - TSD Facility Modifications
This application allows a currently permitted TSD Facility to make modifications to its existing conditions for treating, storing or disposing of hazardous waste.
Hazardous Waste Recycling Permits
- BWP HW 21 - Class A Recycling, with Presumptive Approval
Hazardous waste generators shipping Class A regulated recyclable materials off-site for recycling and facilities recycling these materials must apply for this permit.
- On-Site Class A Recycling Notification Form
Facilities that did not have BWP HW21 - Class A Recycling Permits by February 27, 2004, are required to notify MassDEP before beginning on-site recycling activities, and adhere to specific recycling performance standards thereafter.
- BWP HW 22, 23, 25, 12 - Level I, II, III, and III Precious Metals Recycling
Anyone wishing to recycle hazardous waste, including waste oil, is required to apply for the appropriate permit.
- BWP HW 24, 26 - Level II, III Recycling Permit Renewal or Modification
Anyone wishing to renew or modify an existing Level II or Level III hazardous waste recycling permit is required to submit the appropriate form.
- Periodic Hazardous Waste Reporting
Annual reporting requirements apply to Class A recycling activities subject to permits, as well as to Class B and C recycling. Biennial reporting requirements apply to Large Quantity Generators (LQGs) of hazardous waste as well as Treatment, Storage & Disposal Facilities (TSDFs).
