• Medical Evaluation by a physician.

  • Screening by Coordination of Care Unit by the office nearest the elder's residence. Screening can also be done by the hospital or facility currently treating the individual.

  • Determine the level of care needed: the screening and medical evaluation are reviewed by the Coordination of Care Unit and a determination is made.

  • Decide the appropriate method of payment.

  • Contact the Local Program Area Ombudsman Director for support and suggestions of appropriate facilities.

  • Visit and assess potential long term care facilities, interview staff.

  • Review Admission Agreements of potential facilities.

  • Admission to facility, assessment and care planning.
