After you have narrowed your search for a long term care facility, you should plan on visiting the facilities.
  • Make an appointment to visit with staff that will be involved in your loved one's care (see the checklist pdf format of Assessing LTC - Checklist
).

  • If it is possible to visit unannounced, you will get a better sense of how the facility actually operates on a day-to-day basis.

  • You may wish to utilize the checklist as you visit each facility to aid your decision between two facilities. The checklist is located on this website, or a copy can be sent to you if you contact the Executive Office of Elder Affairs, at 1-800-AGE-INFO or 1-800-243-4636.

  • During your meeting with admissions staff, be sure to request a copy of the admissions agreement prior to admission. This will give you an opportunity to review the contents and prepare questions about the agreement. The Model Nursing Facility Admission Agreement on this website was designed by an ad-hoc group representing various constituencies involved in long-term care. A review of this agreement may aide your understanding of what should be included in an Admission Agreement.
