Your responses are confidential and will be used to help the DPH understand if EI is making a positive difference for your family. Completing this Survey also assists your EI program to improve services families receive.

The Survey and flyer are available in five languages: English, Spanish, Portuguese, Vietnamese, and Haitian Creole.
