Tools for Health Care Coordination

A simple form that direct support professionals or a supervisor would use to record changes in an individual's health status. This form is required for use by provider agencies. Updated September 2007.

This system of forms helps staff or family members prepare for a health care appointment. The procedure for preparation, as well as information gathering tools help to ensure that important health information is recorded and reported.

An electronic form for gathering and organizing an individual's medical history and records in one document so they are available to health care providers and program staff. Program staff or service coordinators enter information in to this form in the HCSIS system. A paper copy can also be kept by the person and used to share information with the health care provider.
