The Executive Office of Health and Human Services (EOHHS) in partnership with the Massachusetts Department of Housing and Community Development (DHCD) received a grant from the U.S. Office of Housing and Urban Development (HUD) to provide housing for Money Follows the Person Demonstration enrollees and other targeted populations. The grant provides project-based rental units and mobile vouchers to eligible populations. The HUD award also designates MFP enrollees as the priority population to receive these vouchers.

  • 811 Project Rental Assistance Year One Report – This report details Massachusetts’ Year One outcomes of the collaboration between the Department of Housing and Community Development and the Money Follows the Person Demonstration in successfully re-housing individuals leaving long term care facilities.
  • 811 Project Rental Assistance Referral Form – This form should be completed for all individuals seeking housing under the 811 Project Rental Assistance (PRA) Grant. The 811 Referral Form certifies eligible applicant information and should only be completed by staff who have received 811 PRA Demo Project training.
  • 811 Project Rental Assistance Pre-application Form – This pre-application is part of the required application materials from the Department of Housing and Community Development when applying for 811 Project-based Subsidies.
  • 811 Project Rental Assistance Application Form – This application should be completed for all individuals seeking housing under the 811 Project Rental Assistance (PRA) Grant.  All forms must be submitted to create a full application including the core documents listed on the Application Checklist.  Core documents include birth certificates, social security cards, photo identifications, proof of income, and proof of assets for all proposed members of the household.
  • General Authorization of Release Form – This form is provided to the Regional Administering Agency that is administering the specific 811 property. The form gives permission for the Regional Administering Agency to verify information submitted that is used to screen for housing eligibility. This form must be submitted during the initial application process, but may be requested again when the applicant is being screened. This form is required as part of a complete application.

This property, located in Haverhill, MA. Hayes at Railroad Square will offer 3 project-based rental assistance units for 811 PRA eligible tenants.

 

Gurney Street/Parcel 25

This property is located in the Mission Hill neighborhood of Boston, MA. Gurney Street/Parcel 25 will offer 3 project-based rental assistance units for 811 PRA eligible tenants.

 

Yarn Works

This property is located in Fitchburg, MA. Yarn Works will offer 2 accessible one bedroom project-based rental assistance units for 811 PRA eligible tenants.
