Helping People Return to the Community
The Money Follows the Person (MFP) Demonstration is a voluntary program that helps elders and people with disabilities who want to move from facility-based care back to the community. MFP provides an array of “Demonstration” services including case management, assistive technology, orientation and mobility, and transitional assistance services designed to help individuals transition from nursing facilities and other long-stay hospital settings to the community.
MassHealth offers two Home- and Community-Based Services (HCBS) waivers, called the Money Follows the Person Waivers (MFP Waivers), which will help individuals who are qualified for the MFP Demonstration to move from a nursing home or long-stay hospital to an MFP-qualified residence in the community and obtain community-based services.
