What is NaviCare HMO? 

NaviCare HMO is a Medicare Advantage Plan and Senior Care Options program that 
combines MassHealth and Medicare benefits. We coordinate your health care needs 
to get you the best possible care. A team of doctors, nurses, social workers 
and other health care professionals work together to build a care plan that’s 
right for you. 

Can I join? 

NaviCare HMO is a health plan for people who: 
• Are 65 or older
• Have Medicare and MassHealth Standard, or just Mass Health Standard alone
• Live at home or in a nursing home
• Live in Worcester County
