Most Massachusetts birthing hospitals use Electronic Birth Certificate software to prepare the Standard Certificate of Live Birth. The is a publication produced by the Registry of Vital Records and Statistics to provide assistance for both hospitals and City and Town Clerks for the legal and statistical aspects of birth record preparation. The EBC Newsletter is used to provide training, to keep the users informed of new updates to the EBC software, and to provide guidelines for updates to birth certificate items. EBC Newsletters for 1999 and 2000 and a content index are provided here for review.
Registering Home Births
Parents should work with their community’s City/Town Clerk to complete a birth certificate for a child born at home.

Report of Fetal Death
The Department of Public Health recently revised the Report of Fetal Death (Form R-304 5/11) in order to be in compliance with federal reporting requirements with the National Centers for Health Statistics (NCHS) which is an agency within the Centers for Disease Control and Prevention (CDC).
The revised Report of Fetal Death (Form R-304 032015) replaces the Report of Fetal Death (Form R-304 5/11) effective November 1, 2014.
