Notice of Public Hearing Concerning Hallmark Health System

Notice is hereby given that the Massachusetts Department of Public Health will conduct a second hearing to permit comment on an application for Determination of Need filed on April 4, 2014 by Hallmark Health System, Inc. (“Hallmark”) a licensed hospital with the following campuses: Melrose-Wakefield Hospital, 585 Lebanon Street, Melrose, MA 02176, and Lawrence Memorial Hospital of Medford, 170 Governors Avenue, Medford, MA 02155. The application seeks a change in the corporate control of Hallmark. Specifically, Partners HealthCare System, Inc., 800 Boylston Street, Suite 1150, Boston, MA 02199 proposes to become the sole corporate member of Hallmark Health Corporation, the current sole member of Hallmark. The hearing will be held on Thursday July 10, 2014 commencing at 6:00 P.M. at “Memorial Hall” a/k/a “Soldiers and Sailors Memorial Building,” 590 Main Street, Melrose, MA 02176. Such hearing shall not be adjudicatory but shall be in the nature of a public forum for the presentation of any comments which may be relevant to consideration by the Department of the need for this project.Such hearing will be jointly conducted with the Division of Health Care Quality under M.G.L. c. 111 s 51G. Persons wishing to make their views known may appear at the hearing or may submit written comments to the Massachusetts Department of Public Health, Determination of Need Program, 250 Washington Street, 6th floor, Boston, MA 02108. Written comments will be accepted up until 5:00 P.M. Monday July 21, 2014. Persons who submitted oral or written comments for the hearing on May 28 need not submit the same testimony for this second hearing.

Notice of Public Hearing Concerning Wing Memorial Hospital

Notice is hereby given that the Massachusetts Department of Public Health will conduct a hearing to permit comment on an application for Determination of Need filed on June 5, 2014 by Wing Memorial Hospital Corporation located at 40 Wright Street, Palmer, MA 01069 with respect to a change in corporate control of Wing Memorial Hospital Corporation (“Hospital”) and its licensed satellite facilities. Baystate Health, Inc. (“Baystate”), located at 280 Chestnut Street, Springfield, MA 01199, proposes to become the sole corporate member of the Hospital pursuant to the terms of an agreement between Baystate and UMass Memorial Health Care, Inc. on behalf of UMass Memorial Hospitals, Inc., the current sole corporate member of the Hospital. The hearing will be held on Thursday July 17, 2014 commencing at 2:00 P.M. in the Palmer High School auditorium, 4105 Main Street, Palmer, MA 01069. Such hearing shall not be adjudicatory but shall be in the nature of a public forum for the presentation of any comments which may be relevant to consideration by the Department of the need for this project. Such hearing will be jointly conducted with the Division of Health Care Quality under M.G.L. c. 111 s 51G. Notice of Public Hearing Concerning Tufts Medical Center

Notice is hereby given that the Massachusetts Department of Public Health will conduct a  hearing to permit public comment on an application for Determination of Need filed on May 16, 2014 by Tufts Medical Center, Inc. (“Tufts”), a licensed hospital located at 800 Washington Street, Boston, MA 02111, with respect to a change of ownership of the hospital.  Tufts Medical Center Parent Inc., the sole corporate member of Tufts, proposes to form a new entity with Circle Health, Inc., the sole corporate member of Lowell General Hospital, Inc.(“LGH”) located at 295 Varnum Ave., Lowell, MA 01854.  Tufts Circle Health, Inc. will become the parent corporation of both Tufts and LGH.  The hearing will be held on Thursday July 31, 2014 commencing at 2:00 P.M at the Massachusetts Department of Public Health, 2nd Floor, 250 Washington Street, Boston, MA 02108.  The hearing will not be adjudicatory but shall be in the nature of a public forum for the presentation of any comments which may be relevant to consideration by the Department of the need for this project.  The hearing will be jointly conducted with the Division of Health Care Quality under M.G.L. c. 111 s 51G.  Persons wishing to make their views known may appear at the hearing or may submit written comments to DPH-DoN@massmail.state.ma.us or to Massachusetts Department of Public Health, Determination of Need Program, 250 Washington Street, 6th floor, Boston, MA 02108.  Written comments will be accepted up until 5:00 P.M. Notice is hereby given that the Massachusetts Department of Public Health will conduct a hearing to permit public comment on an application for Determination of Need filed on May 16, 2014 by Lowell General Hospital (“LGH”), a licensed hospital located at 295 Varnum Avenue, Lowell, MA 01854, with respect to a change of ownership of the hospital.  Circle Health, Inc., the sole corporate member of LGH, proposes to form a new entity with Tufts Medical Center Parent, Inc., the sole corporate member of Tufts Medical Center (“Tufts”) located at 750 Washington Street, Boston, MA 02111.  Tufts Circle Health, Inc. will become the parent corporation of both LGH and Tufts.  The hearing will be held on Wednesday July 30, 2014 commencing at 2:00 P.M at UMass Lowell in the O’Leary Library, 2nd Floor, 61 Wilder Street, Lowell, MA 01854.  The hearing will not be adjudicatory but shall be in the nature of a public forum for the presentation of any comments which may be relevant to consideration by the Department of the need for this project.  The hearing will be jointly conducted with the Division of Health Care Quality under M.G.L. c. 111 s 51G.  Persons wishing to make their views known may appear at the hearing or may submit written comments to DPH-DoN@massmail.state.ma.us or to Massachusetts Department of Public Health, Determination of Need Program, 250 Washington Street, 6th floor, Boston, MA 02108.  Written comments will be accepted up until 5:00 P.M.
