For youth and young adults there are developmentally appropriate readiness self-assessments to encourage self-determination and independence; tips for communicating health needs with parents and providers, including managing chronic or medically complex needs in a college setting; and peer support and advocacy groups.
For parents and caregivers there are youth and parent readiness assessments; strategies to support developmentally appropriate medical self-management skills in youth; and parent support and advocacy groups.
For health care providers there are resources to assess medical home readiness for individual, group and network practices; CQI tools and surveys including the Six Core Elements of Health Care Transition; transition readiness algorithms; web-based CME opportunities; along with provider focused guides, videos and manuals.
