Data File(s)The Division of Health Professions Licensure is pleased to offer public information in electronic format. This process allows us to provide you with the detailed, accurate information you need quickly and efficiently.

The Division will provide certain public information in accordance with the Public Records Law MGL c4, s7; MGL c66, s10 and 950 CMR 32.00.

The following public information about licensees is available:

  • Full Name
  • License Number
  • License Type
  • License Status
  • Expiration Date
  • Street Address
  • City, State, and Zip Code
  • County
  • Issue Date

A PC-compatible CD-Rom containing a text file will be mailed to customers for a fee ($30 per selected box in the request form). Each data file contains all of the records for the chosen profession(s).
